Passports

Passports for Persons 14 years and older

To obtain the first passport for a person 14 years or older, or if your passport has been expired for more than 5 years, the applicant must apply in person to the U.S. Embassy, Apia.

You will need to:

  1. Complete "Application for Passport", Form DS-11.  Please note that some of the instructions apply only to U.S. citizens residing in the U.S.
  2. Provide Two Passport Photos
