Extra pages:

Effective September 26, 2005, as an additional security enhancement, Passport Services will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error will apply for replacement passports - Media Note ( PDF Document size: 13Kb *PDF document).

To add extra pages to your passport, you will need to bring your passport to the following address:

Matafele
Apia
 

This service can be done within a day.  There is no fee to add extra pages to your passport.

Passport name change:

If your passport was issued less than 12 months ago please come to the Embassy with the following:

  • A U.S. Passport Re-Application form DS-5504
  • The passport that needs the change
  • The original of your marriage certificate or your name-change court decree

If your passport was issued more than 12 months ago please submit:

  • An Application for Passport Renewal form DS-82
  • The passport that needs the change
  • The original of your marriage certificate or your name-change court decree
  • The applicable fee(s).  If the requested data change is due to a printing error by Passport Services, there will be no charge.

Photocopies and notarized copies are not acceptable for passport purposes.

Please allow a minimum of one month to process your application.
