Passport for minors under age 14

To obtain a passport for a minor under age 14, the applicant must apply in person to the U.S. Embassy, Apia. 

You will need to:

1. Complete "Application for Passport", Form DS-11.  Please note that some of the instructions apply only to U.S. citizens residing in the U.S.
2. Present proof of U.S. citizenship of minor
3. Present evidence of minor's relationship to parents/guardians
4. Provide parental identification of minor
5. Present proof of identity of minor
6. Present Parental Application Statement of Consent form DS-3053
7. Provide two passport photos of minor
8. Pay the applicable fee
9. Provide a Social Security Number of minor.
