Passports

Damaged or Mutilated Passport

To obtain a new passport, you must apply in person at the U.S. Embassy in Apia.

You will need to:

  1. Complete "Application for Passport", Form DS-11.  Please note that some of the instructions apply only to U.S. citizens residing in the U.S.
  2. Present damaged passport
  3. Provide Two Passport Photos
  4. Complete a DS-64: Statement of damagePDF Document size: 115Kb *PDF document).
