Lost or Stolen Passport

What to do:

  • Please notify the nearest Samoan police station of the loss/theft.
  • Inform the U.S. Embassy, Apia of the loss/theft by telephone at 21631.
  • Leave a message on our voice mail if necessary. 

If you decide not to apply for a new passport immediately, you may report your lost or stolen passport by completing Form DS-64PDF document size: 115Kb *PDF document) and bringing it into the U.S. Embassy in Apia.

To obtain a new passport, you must apply at the U.S. Embassy in Apia.

You will need to:

  1. Complete "Application for Passport", Form DS-11.  Please note that some of the instructions apply only to U.S. citizens residing in the U.S.
  2. Complete Form DS-64: Statement Regarding Lost Or Stolen Passport ( PDF Icon *PDF document)
  3. Present a Samoan Police Report regarding the passport loss.
