Adult Passport Renewal

To renew your passport by mail using Form DS-82, there are 4 criteria:

  1. You can submit your most recent passport book and/or U.S. passport card with this application AND
  2. If you were at least 16 years old when your most recent passport book and/or U.S. passport card was issued AND
  3. If you were issued your most recent passport book and/or U.S. passport card less than 15 years ago AND
  4. If  you use the same name as on your most recent passport or; since your passport was issued, you have had your name changed by marriage or court order and can submit proper documentation to reflect that name-change.

If any of these do not apply, use Form DS-11 and see instructions for adult passports.
All passport renewals must include the following:

Your previous passport will be returned to you with your new passport.

If your name has changed since your passport was issued:

  • Enclose the original of the legal document specifying your name change (e.g. marriage certificate, divorce decree, adoption decree, or court order).
  • No photocopies accepted.
